DEATH REGISTRATION

A Death Certificate is an official record that documents the details and circumstances of the deceased person.

Requirements

Barangay Certificate

Health Certificate

Valid ID, Birth Certificate, or Marriage Contract of the deceased

Valid ID of the representative

Processing Fees

Registration Fee – ₱260

ONLINE PROCESS

Complete these steps through our E-Services portal

    Log in to E-Services.

  2. 2

    Select Death Registration.

  3. 3

    Fill out the required information of the deceased.

  4. 4

    Upload the documentary requirements.

  5. 5

    Submit the application.

  6. 6

    Wait for validation and instructions via email or check updates on the e-service dashboard.

  7. 7

    Submit original documents onsite once instructed.

  8. 8

    Pay corresponding registration fee of Php 260 at the Treasurer's Office cashier.

  9. 9

    Upon receiving the official receipt, the clerk will release the document to the applicant.

ONSITE PROCESS

Walk-in to complete these steps

    Fill out the Certificate of Death form and submit it to the Civil Registry Department together with other requirements.

  2. 2

    The receiving clerk will evaluate if the documents are complete and issue an order of payment.

  3. 3

    Pay the corresponding Registration Fee of Php 260 at the Treasurer's Office cashier.

  4. 4

    Upon receiving the official receipt, the clerk will release the document to the applicant.
